The development of exoskeletons is currently a lengthy process full of challenges. We are proposing a framework to accelerate the process and make the resulting exoskeletons more user-centered. The needed accomplishments in science are described in an effort to lay the foundation for future research projects. Since the early 2000s, exoskeletons have been discussed as an emerging technology in industrial, medical, or military applications. Those systems are designed to support people during manual tasks. At first, those systems lacked broad acceptance. Many models found their niches in ongoing developments and more diverse systems entering the market. There are still applications that are in dire need of such assistance. Due to the lack of experience with body-worn robotics, the development of such systems has been shaped by trial and error. The lack of legacy products results in longer development times. In this paper, a process to generate a framework is presented to display the required research to enable future exoskeleton designers. Owing to their proximity to the user’s body, exoskeletons are highly complex systems that need sophisticated subsystems, such as kinematic, control, interaction design, or actuators, to be accepted by users. Due to the wide variety of fields and high user demands, a synchronized multidisciplinary effort is necessary. To achieve this, a process to develop a modular framework for exoskeleton design is proposed. It focuses on user- and use-case-centered solutions for matching kinematics, actuation, and control. To ensure the usefulness of the framework, an evaluation of the incorporated solutions is required.

This review meta-analysis combines and compares the findings of previously published works in the field of soft wearable robots (SWRs) that provide active methods of actuation for assistive and augmentative purposes. A thorough investigation of major contributions in the field of an SWR is made to analyze trends in the field focused on fluidic and cable-driven systems, prevalent and successful approaches, and identify the future direction of SWRs and active actuation strategies. Types of soft actuators used in wearables are outlined, as well as general practices for fabrication methods of soft actuators and considerations for human–robot interface designs of garment-like exosuits. An overview of well-known and emerging upper body (UB)- and lower body (LB)-assistive technologies is categorized by the specific joints and degree of freedom (DoF) assisted and which actuator methodology is provided. Different use cases for SWRs are addressed, as well as implementation strategies and design applications.

Wearable electronics are an essential direction for the future development of smart wearables. Among them, the battery life of wearable electronics is a key technology that limits their development. The proposal of self-powered wearable electronics (SWE) provides a promising solution to the problem of long-term stable working of wearable electronics. This review has made a comprehensive summary and analysis of recent advances on SWE from the perspectives of energy, materials, and ergonomics methods. At the same time, some representative research work was introduced in detail. SWE can be divided into energy type SWE and sensor type SWE according to their working types. Both types of SWE are broadly applied in human–machine interaction, motion information monitoring, diagnostics, and therapy systems. Finally, this article summarizes the existing bottlenecks of SWE, and predicts the future development direction of SWE.

The treatment and evacuation of people with lower limb fractures in austere environments presents unique challenges that assistive exoskeletal devices could address. In these dangerous situations, independent mobility for the injured can preserve their vital capabilities so that they can safely evacuate and minimize the need for additional personnel to help. This expert view article discusses how different exoskeleton archetypes could provide independent mobility while satisfying the requisite needs for portability, maintainability, durability, and adaptability to be available and useful within austere environments. The authors also discuss areas of development that would enable exoskeletons to operate more effectively in these scenarios as well as preserve the health of the injured limb so that definitive treatment after evacuation will produce better outcomes.

The large-scale adoption of occupational exoskeletons (OEs) will only happen if clear evidence of effectiveness of the devices is available. Performing product-specific field validation studies would allow the stakeholders and decision-makers (e.g., employers, ergonomists, health, and safety departments) to assess OEs’ effectiveness in their specific work contexts and with experienced workers, who could further provide useful insights on practical issues related to exoskeleton daily use. This paper reviews present-day scientific methods for assessing the effectiveness of OEs in laboratory and field studies, and presents the vision of the authors on a roadmap that could lead to large-scale adoption of this technology. The analysis of the state-of-the-art shows methodological differences between laboratory and field studies. While the former are more extensively reported in scientific papers, they exhibit limited generalizability of the findings to real-world scenarios. On the contrary, field studies are limited in sample sizes and frequently focused only on subjective metrics. We propose a roadmap to promote large-scale knowledge-based adoption of OEs. It details that the analysis of the costs and benefits of this technology should be communicated to all stakeholders to facilitate informed decision making, so that each stakeholder can develop their specific role regarding this innovation. Large-scale field studies can help identify and monitor the possible side-effects related to exoskeleton use in real work situations, as well as provide a comprehensive scientific knowledge base to support the revision of ergonomics risk-assessment methods, safety standards and regulations, and the definition of guidelines and practices for the selection and use of OEs.

Elastic textiles play a critical role in passive wearable solutions for musculoskeletal load management in both passive exosuits and resistance clothing. These textiles, based on their ability to stretch and retract, can exhibit ambivalence in their load-modulating effects when used in occupational, rehabilitation, exercise, or everyday activity settings. While passive exosuits and resistance garments may appear similar in design, they have opposing goals: to reduce the musculoskeletal load in the case of exosuits and to increase it in the case of resistance clothing. Despite this intrinsic connection, these two approaches have not been extensively linked together. This review aims to fill this gap by examining the common and distinct principles of elastic textiles in passive exosuits and resistance clothing, shedding light on their interactions and the complex dynamics of musculoskeletal load systems. The effectiveness of different designs in passive exosuits that mimic musculoskeletal function and resistance clothing that increase the workload for strength training are critically reviewed. Current challenges in practical implementation and opportunities to improve critical issues, such as preload, thermal comfort, skin friction, and donning and doffing are also highlighted.

Inertial measurement units (IMUs) have proven to be valuable tools in measuring the range of motion (RoM) of human upper limb joints. Although several studies have reported on the validity of IMUs compared to the gold standard (optical motion capture system, OMC), a quantitative summary of the accuracy of IMUs in measuring RoM of upper limb joints is still lacking. Thus, the primary objective of this systematic review and meta-analysis was to determine the concurrent validity of IMUs for measuring RoM of the upper extremity in adults. Fifty-one articles were included in the systematic review, and data from 16 were pooled for meta-analysis. Concurrent validity is excellent for shoulder flexion–extension (Pearson’s r = 0.969 [0.935, 0.986], ICC = 0.935 [0.749, 0.984], mean difference = −3.19 (p = 0.55)), elbow flexion–extension (Pearson’s r = 0.954 [0.929, 0.970], ICC = 0.929 [0.814, 0.974], mean difference = 10.61 (p = 0.36)), wrist flexion–extension (Pearson’s r = 0.974 [0.945, 0.988], mean difference = −4.20 (p = 0.58)), good to excellent for shoulder abduction–adduction (Pearson’s r = 0.919 [0.848, 0.957], ICC = 0.840 [0.430, 0.963], mean difference = −7.10 (p = 0.50)), and elbow pronation–supination (Pearson’s r = 0.966 [0.939, 0.981], ICC = 0.821 [0.696, 0.900]). There are some inconsistent results for shoulder internal–external rotation (Pearson’s r = 0.939 [0.894, 0.965], mean difference = −9.13 (p < 0.0001)). In conclusion, the results support IMU as a viable instrument for measuring RoM of upper extremity, but for some specific joint movements, such as shoulder rotation and wrist ulnar-radial deviation, IMU measurements need to be used with caution.

The science and technology of wearable robots are steadily advancing, and the use of such robots in our everyday life appears to be within reach. Nevertheless, widespread adoption of wearable robots should not be taken for granted, especially since many recent attempts to bring them to real-life applications resulted in mixed outcomes. The aim of this article is to address the current challenges that are limiting the application and wider adoption of wearable robots that are typically worn over the human body. We categorized the challenges into mechanical layout, actuation, sensing, body interface, control, human–robot interfacing and coadaptation, and benchmarking. For each category, we discuss specific challenges and the rationale for why solving them is important, followed by an overview of relevant recent works. We conclude with an opinion that summarizes possible solutions that could contribute to the wider adoption of wearable robots.

The aging population is now a global challenge, and impaired walking ability is a common feature in the elderly. In addition, some occupations such as military and relief workers require extra physical help to perform tasks efficiently. Robotic hip exoskeletons can support ambulatory functions in the elderly and augment human performance in healthy people during normal walking and loaded walking by providing assistive torque. In this review, the current development of robotic hip exoskeletons is presented. In addition, the framework of actuation joints and the high-level control strategy (including the sensors and data collection, the way to recognize gait phase, the algorithms to generate the assist torque) are described. The exoskeleton prototypes proposed by researchers in recent years are organized to benefit the related fields realizing the limitations of the available robotic hip exoskeletons, therefore, this work tends to be an influential factor with a better understanding of the development and state-of-the-art technology.

Specialists globally employ various clinical scales and instruments to assess balance, gait, and motor functions in children with cerebral palsy (CP). Selecting appropriate assessment tools is essential for planning studies, developing effective treatment strategies, and tracking clinical outcomes. Given the diversity in assessment needs – whether evaluating dynamic, functional, or static balance – there is a need to identify the most suitable tools for each aspect. Therefore, the primary objective of this review is to critically analyze current clinical and instrument-based assessment methods in the literature to determine the most effective approaches for pediatric CP. This systematic review retrieved 1,812 papers, of which only 23 met the inclusion criteria and presented assessment methods for evaluating balance and motor functions in pediatric CP. These methods were further organized into clinical and instrument-based assessment groups. Among clinical examinations, the Pediatric Balance Scale and Gross Motor Function Measures were considered gold standards and featured in eight studies. In contrast, postural sway measured with the Biodex Balance System, Gait Stability Indices from the GAITRite system, and EMG sensing were the predominant instrument-based observations. Despite this variety, a consensus on the best assessment methods remains lacking. This review highlights the potential of integrating AI-driven metrics that combine clinical and instrument-based data to enhance precision and individualized care. Future research should focus on creating integrated, individualized profiles to better capture the unique capabilities of children with CP, enabling more personalized and effective intervention strategies.

Individuals with cerebral palsy (CP) experience significant impairments in lower limb mobility, which severely limit their daily activities and overall quality of life. Robotic exoskeletons have emerged as a cutting-edge solution to assist in the rehabilitation of individuals with CP by improving their motor functions. This systematic review, conducted following PRISMA guidelines, critically evaluates lower limb robotic exoskeletons specifically designed for individuals with CP, focusing on their design, rehabilitation interfaces, and clinical effectiveness. The review includes research papers published between 2010 and 2024, analyzing 30 lower limb exoskeletons reported in 57 papers. We analyze each exoskeleton, focusing on its technological features, user experience, and clinical outcomes. Notably, we identify a trend in which researchers are increasingly adapting exoskeleton functions to the specific needs of individual users, facilitating personalized rehabilitation approaches. Additionally, we highlight critical gaps in current research, such as the lack of sufficient long-term evaluations and studies assessing sustained therapeutic impacts. While ease of use remains crucial for these devices, there is a pressing need for user-friendly designs that promote prolonged engagement and adherence to therapy. This comprehensive review of existing gait rehabilitation exoskeleton technologies aimed to inform future design and application, ultimately contributing to the development of devices that better address the needs of individuals with CP and enhance their motor functions and quality of life.
